When a Provider is no longer eligible to provide subsidy care, or moved location outside your service area, etc., you may terminate the Provider record in CareControl.

Start by accessing the Provider record to be terminated.

Locate the Action button at the top right corner and select Change Provider Status.

Select the Status as Terminated and specify the Terminate Effective Date.

Once selecting Done to finalize this action, the Provider status will immediately reflect as Terminated.

Under the "Provider Info" Tab, the End Date will be automatically display the selected Terminate Effective Date.

If the current Provider record has active care schedules, you will also need to generate a Task to the Case Managers to update the care schedules for all families / children following the Provider Termination.
