CareControl will guide you with the provider entry:
1. The first step is to select whether the Provider Category is Individual, Corporation or Partnership based on their W-9. This category will decide the next information available:
a. When Provider Category is Individual: Provider’s Last name and First name will be required entries, with both SSN and EIN available.
b. When Provider Category is Corporation or Partnership: Provider (Business) Name will be a required entry, with only the EIN available.
2. The next step is to enter the demographic information in 4 sections:
a. Basic Info: This section contains the basic information for the Provider, including a Provider Type selection which allows you to further breakdown the different types such as:
i. Exempt

- Exp-In-Non-Rel: Provider is license-exempt, in-home, non-relative to the child
- Exp-In-Rel: Provider is license-exempt, in-home, relative to the child
- Exp-Out-Non-Rel: Provider is license-exempt, in-home, non-relative to the child
- Exp-In-Non-Rel: Provider is license-exempt, in-home, non-relative to the child
ii. Licensed

- Licd-Center: Provider is a licensed center
- Licd-Exp-School: Provider is a license-exempt but is a school setting
- Licd-Small-Family: Provider is a licensed small family child care home with less than 6 children
- Licd-Large-Family: Provider is a licensed large family child care home with less than 12 children
b. Contact Info: This section contains the basic contact information for the Provider, with their email address, work phone, home phone, and cell phone.
c. Address Info: This section contains the address information for the Provider, with their physical address and mailing address.
d. Tax Info: This section contains the tax information for the Provider with the SSN and/or EIN available based on the selected Category in the previous step.
3. Once completed, click Save to create the provider record.
a. CareControl will redirect you to the newly created Provider record with the Provider ID automatically assigned.
b. Newly created Provider will be under “Requested” status. You may review Provider information and update all other details as needed prior to approving the Provider.
c. Based on the Provider Category and Provider Type selected, the Provider record will display differently:
i. Exempt:

- Exempt Providers will require a Trust Line record, which will have a Trust Line tab available alongside other Tabs to capture all necessary information.
ii. Licensed:

- Licensed Providers will require License, Registration Fee, Holidays which will have the 3 corresponding Tabs available along side other Tabs to capture all necessary information. You can edit the Provider Info for Licensed Providers by hovering over the Details tab to show the Provider Info and Contacts sub-tabs.
