Access CareConnect Provider mobile app (iOS and Android) or CareConnect Provider Web.

There are several built-in reports available in CareConnect to manage all students-related information for your program that are:

1. 801A
2. Student Immunization Record
3. Contact & PIN
4. Demographic
5. Enrollment Activity
6. Family Contacts Details
7. FamilyPortal Linkage Details
8. Family Recertification Date
9. Immunization Assessment
10. Physician Details
11. Tags Details

Each of the reports listed above can be printed or exported as a PDF and/or an Excel spreadsheet file. If you're unable to view the reports clearly, either click on the + button to Zoom in on the report or click on Save as --> XLSX (Excel spreadsheet) to export the report out of CareConnect.

To find these reports, navigate to Students Module --> All will be listed under the Reports section. Below are more details for each:

1. 801A 

This report's availability is depending on your program and whether or not you operate subsidized programs for your families. If yes, the 801A report in CareConnect directly reflects state requirements and is formatted for you to submit the report directly to the CDMIS website.

2. Student Immunization Record

This report is a quick way to batch-print all student's immunization records rather than printing them individually per student in the family's record.

You can filter for the desired Student Name, or filter for multiple students using Sites & Classrooms, Student Status, the Care Period, or the different Programs.

3. Contact & PIN

This report can be used to locate all contact information together with their PIN (Personal Identification Number) for electronic attendance using either the kiosk or contactless attendance method. 

You can filter for a specific Site & Classroom, Service Month, or Program.

4. Demographic

This report provides all demographic details for all students enrolled in your program, such as profile details, schedule information, parent information, etc.

You can filter for a specific Site, Age Range, and specific Programs.

5. Enrollment Activity

This report lists the total enrollment count per site and program. You can view counts for students that are beginning enrolled, newly enrolled, end enrolled, or continuing enrolled.

You can filter for specific Sites, Dates (monthly), and programs.

6. Family Contacts Details

This report gives all the students' family information in a sheet. Emergency contact details such as phone number, relationship, and address of the contacts are provided.

You can filter for the desired Student Name, or filter for multiple students using Sites & Classrooms, Student Status, the Care Period, Student Tag, or the different Programs.

You can click on Save As --> XLSX to download and view it in Excel. You can also click the + button to view the information in a larger font.

7. FamilyPortal Linkage Details

This report provides parents' details for FamilyPortal Linkage such as linked account, online payment authorization, auto pay authorization, Payment method, and Last Login date.

You can filter for the desired Student Name, or filter for multiple students using Sites & Classrooms, Student Status, the Care Period, Student Tag, or the different Programs.

8. Family Recertification Date

This report provides information for all families that have an upcoming recertification date. This will be needed for subsidized families if you need to view a record of families' recertification date, student's status and enrolled site & classrooms, program, and care period.

You can filter for the desired Student Name, or filter for multiple students using Sites & Classrooms, Student Status, the Care Period, Student Tag, or the different Programs.

9. Immunization Assessment

This report allows you to view all students' immunization records in a list sorted by different vaccine types. Click here to view a more in-depth guide about the different immunization reports on CareConnect. 

You can filter this report by Site and Student's age in a range as of a certain date (Start Year, Start Month, End Year, End Month).

Report preview is directly available in CareConnect with options for PDF or Excel spreadsheet files. It is recommended to download the file to get a full view of the record by selecting Save As --> XLSX.

10. Physician Details

This report gives detailed information regarding students, their parents, and their physician information.

You can filter this report by Site, Student's age in a range as of a certain date (Start Year, Start Month, End Year, End Month), and students enrolled Program.

11. Tags Details

This report gives general students' information including their tags. You can use this report to view specific students with certain allergies, diet restrictions, or possible requests from parents (ex: dropping off students early, taking medicine at a specific time, etc).

You can filter for the desired Student Name, or filter for multiple students using Sites & Classrooms, Student Status, the Care Period, Student Tag, or the different Programs.
