Eligibility
Access CareConnect Provider mobile app (iOS and Android) or CareConnect Provider Web.
Note: If the family/student is enrolling under a Subsidy program, do not skip this section
The Eligibility tab allows you to add pertinent information for the family, parent, and student following Eligibility requirements for generating the EESD 9600, NOA forms, and 801A reporting. Any data entries within the Eligibility tab will need to be manually saved. The 3 subsections are Student, Parent, and, Family.
Student
All students in the family will be listed to specify their Eligibility details. Click on + Add to add a Category, Language, English Learner, or IEP/IFSP. From here, you can select the Student and Start/End date for the section selected.
Tip: Make sure to click on theicon to save. Fields marked below with an *asterisk are required; data validation checks may trigger if enabled by your administrator.
Parent
Both Parents A and B will be listed to specify their Eligibility Details including the Needs activity. Make sure to click on Save after making any changes.
- Select + Add Activities to include a new activity record. From here, you may specify:
- Need Type: select an activity for the parent
- Name: fill out the entity/contact name for the activity, if applicable
- Period: enter the activity date period
- Variable: whether it is a variable schedule or not
- Address, Zip, City, State: fill out the entity address information for the activity, if applicable
- Comment: enter any comments for the current family income/fee information
- + Add Schedule: Add the Scheduled hours, if applicable
Family
Any Eligibility required details for the entire family will be listed here, including the overall Family income.
To add the Family Income:
- Select + Add Income to add a new family income record. You may specify:
- Start Month: This is the start month the income and fee information will be effective. This is a required field
- End Month: The last month the income and fee information will be effective

Monthly Income: Enter the total monthly income for the entire family. Selecting the Calculator Icon will open an Income Calculation Tool
- Select which parent the income applies to
- For wage income, select Wages (selecting again will add another income of that type)
- Enter the Start Date and End Date (if applicable)
- Select the Pay Schedule and if it is a 10-Month Formula
- Enter the pay date and the amount. Select add income to add additional Pay Date and amounts
- Select Calculate when complete
- For other incomes, select Other Incomes (selecting again will add another income of that type)
- Enter the Start Date and End Date (if applicable)
- Select Income Source, Income Type, Frequency
- Enter Amount
- Select Calculate when complete
- Family Size: Enter the total family size for the entire family
- Family Fee (PT) and (FT) will auto-populate from the system once the Family Income & Size are entered
- Comment: Enter any comments for the current family income/fee information
At the end of every subsection of the Eligibility page, there are two options:

Verify: This is a feature to help check any missing Eligibility details
- Print: To print the Application or the Notice of Action (NOA) form
