Access CareConnect Provider mobile app (iOS and Android) or CareConnect Provider Web.
When you click on a Student's Hyperlink Name, you will be in the Family's Profile. From here, you will have access to the Student Profile, Eligibility, Messages, Documents, Notes, and Logs. Here is an overview of each section.
Student Profile
In the Student Profile, CareConnect will always display the selected student. For each student, there are 4 different subsections:
- Personal Info - Here you'll find personal details for the student including Profile Pictures, pertinent information such as allergies, medications, and more
- Schedule - Here you'll be able to review the care approval for the student
- Immunization - This allows you to track the immunization records and able to print them out
- Physicians - Here you'll be able to track the physician records and TB screening information for the student
Eligibility
Note: If the family/student is enrolling under a Subsidy program, do not skip this section
The Eligibility tab allows you to add pertinent information for the family, parent, and student following CDE requirements for generating the CD9600, NOA forms, and 801A reporting. Any data entries within the Eligibility tab will need to be manually saved. There are 3 subsections in the Eligibility tab Student, Parents, and Family.
Messages
The Messages tab allows you to view any message(s) sent to the family contact(s) from the Message Module. You can search through messages or filter by dates to view specific messages. If you ever need to refresh this section, please click on the Refresh icon. You may export all of the messages onto an Excel Sheet by clicking on the Export icon. If you need to filter to view only some columns, please click on the Column Chooser icon .
Documents
The documents tab allows you to upload documents to CareConnect and send them to a parent’s FamilyPortal account. If your organization or parent is not using FamilyPortal, then this section can be used as a placeholder to keep documents related to the family. If you ever need to refresh this section, please click on the Refresh icon. Feel free to export this information onto an Excel Sheet by clicking on the Export icon. If you need to filter to view only some columns, please click on the Column Chooser icon .
Notes
The notes tab allows you to add and track any family information. When you want to add a note, simply click on + Add. You will be asked to set any topics you want to tag your note, then add the note content below. Once entered, you can click on Done to add this note to the Family Notes. To delete a note or multiple notes, select the notes, then click on Delete. If you ever need to refresh this section, please click on the Refresh icon. Feel free to export this information onto an Excel Sheet by clicking on the Export icon. If you need to filter to view only some columns, please click on the Column Chooser icon .
Logs
The logs tab is a system log that will store all actions/changes to the family. Use this to know who made what changes at what times. Feel free to export this information onto an Excel Sheet by clicking on the Export icon.
