Access CareConnect Provider mobile app (iOS and Android) or CareConnect Provider Web.
Once the student is located from the All Students list under the Students Module --> Click on the Student's Name hyperlink to open the details.
From there you will have access to the Family Overview which includes the Student Profile, Eligibility, Messages, Documents, Notes, and Logs.
By default, CareConnect will always display the selected student in the Student Profile. For each student, there are 4 different tabs:
- Personal Info: Here you'll find personal details for the student including Profile Pictures, pertinent information such as allergies, medications, and more
- Schedule: Here you'll be able to review the care approval for the student
- Immunization: This allows you to track the immunization records and able to print them out
- Physicians: Here you'll be able to track the physician records and TB screening information for the student
Student Profile
In the Student Profile, you will have immediate access to view and review information from that student. Feel free to add a profile picture for the students. You will also have 3 icons below the student's profile photo:
- Move Student - Move the student to a new or existing family by family ID. This allows you to connect students with siblings to show up in the same family.
- Attendance - Redirect to the student's attendance.
- Billing - Redirect to the student's family ledger.
If there are other siblings, you may select directly the sibling for reviewing and/or updating the sibling’s details information. If you need to add any additional Siblings to the Student Profile, you can do so by clicking on + Add Sibling.
Personal Info
In the Personal Info tab, you will have access to Student's Basic Information, Parents / Guardians, Other Contacts, and Additional Authorized Pickup.
Tips: if there is any incorrect information, you may click on Edit. Once any edits have been made, click Save to see the updated information on the student's profile.
In Parents / Guardians, you may:
- Click on + Add Parent to add Parent A or Parent B
- Click on the pencil icon to edit any contact
In Other Contacts, you may:
- Click on + Add Contact to add a contact
- Click on the pencil icon to edit any contact
In Additional Authorized Pickup up, you may:
- Click on + Add Pickup to search by family ID to link
- Click on the trash icon to remove the linkage if it is no longer needed
If needed, you can regenerate a brand new PIN for any contact by clicking on Re-PIN. You can also email their PIN information along with a helpful guide on how to use their PIN by clicking on the Email Sign-in Guide or you can simply send their PIN information through a text message by clicking on Text Sign in PIN.

Schedule
- Schedule shows the care approval for the student (if any)
- Select + Add Schedule for adding new schedules
- Click on the pencil icon under the Action column for updating the current schedule information
- Click on the copy iconunder the Action column for copying the existing schedule to create a similar new schedule
Immunization
The immunization tab allows you to track immunization records (if any). This also gives you an insight into vaccines that students still need to receive and as well as any outdated vaccination records.
- Click on Edit to add the immunization record for the student
- Click on Print for printing the California School Immunization Record form
Physicians
The physician tab allows you to track the physician records and TB screening information for the student (if any). You can also add the student's insurance information.
- Click on Edit to add the physician information
- Click on Edit to add any TB Screening information
