An agreement allows for the tracking of a request from a family for a specific provider. With the agreement tab within the family record, the system can inform the user if the provider is already approved through the agency, or add the Provider into the system if they are not approved yet.
Adding a New Agreement for an Approved Provider
- From the Family Record, navigate to the Agreements Tab. To add a new agreement, click the Add button.
- The system will prompt you to search for the provider that the family is requesting. Enter in the providers information here. This is to check if the provider has been approved for services previously.
- After pressing search, the system will present a list of providers that match the search criteria. If the provider has already been approved for services, they can be selected here.
- After selecting the approved provider, click Use Selected Provider. The system will then prompt you to enter Agreement information, such as:
- Start Date of Agreement
- End Date
- Program Services are Approved for
- Relationship of Provider to Family
- Checkbox for Relative
- Checkbox for In Home Services
- Requested Date
- Requested Reason
- Once information has been entered, clicking Submit will complete the process, adding this agreement to the Provider Record, as well as an entry to track from the Family Record.
Adding a New Agreement for a New Provider
- When searching for the provider, if the provider has not been approved for services, no entry will appear. From step 3 of the previous section, click Add New Provider.
- The system will now prompt you to add the information for the new Provider. For more information on this step, please refer to the following Article.
- Once the Providers information has been entered, add the agreement information, as with Step 4 of the previous section.
Updating Agreement Status
Whenever the status is changed within the provider agreement, the system will prompt the user to send a task to both the creator of the agreement and the case owner. This will allow the original creator of the agreement to know when the provider has been approved for services, has been denied for services, or been terminated from providing service.
