CareConnect offers the ability to generate the PLIS report for your CSPP students. At the time of enrolling a student and family into CareConnect, you will be able to gather, generate, then export to CDMIS within the Eligibility tab of the family record.
Pre-Configuration:
Before generating your PLIS report, please check and update any of the details within your sites and classrooms.
1. CDS Code, Site License, and Site Address:
Please visit your Admin module → Sites and enter your LEA Provider CDS Code, site's License information, or site's address.
Admin module → Sites → Profile → Click Edit → enter the LEA Provider CDS Code or address and Save
If you have the information for the Site's License, please visit your Admin module → Sites → Select your site → License
Click +New to add License information and Save
2. Classroom Configuration:
Admin module → Sites → Classrooms → Click the Classroom name→ Click Edit → enter the following 4 fields with their appropriate details:
- Language(s) Used - Lead Teacher
- Language(s) Proficient - Lead Teacher
- Language(s) Used
- Language(s) Proficient
- Language Program Types
During Enrollment:
CareConnect offers the ability to digitally send and receive documents during the enrollment process. This can help streamline and reduce the enrollment processing time by having parents digitally fill in the (1) Family Language Instrument and conduct the (2) Family Language and Interest Interview through the Documents Module.
1. Family Language Instrument:
After sending and receiving back the Family Language Instrument form, you will be able to directly enter the information into CareConnect.
From the Student's Profile → Eligibility tab → Student Subtab → in the Basic section → click the pencil icon to add the PLIS Start Date, Date of the Instrument, the value for the Dual Language Learner, the value for the DLL Teacher Designation and click Save.
The Start Date is the child's initial subsidy start date; the PLIS Start Date is the child's first day in CSPP. These dates may or may not match depending on when the child entered CSPP.
2. Family Language and Interest interview:
You'll now be able to proceed to conduct and record information for the Family Language and Interest interview if family conditions are met.
NOTE: To learn more about how to fill out documents in person, click here.
Once the Family Language and Interest interview form has been completed, 4 values will need to be manually entered in the Family's Eligibility section.

- Home Language
- The families:
- Written Communication Preference
- Verbal Communication Preference
To add the student's Most Used Language, navigate to the Student's Profile → Eligibility tab → Student Subtab → in the Most Used Language section → click +Add Most Used Language or click the pencil icon to enter the start, end date, and Language.

To add the student's Home Language, navigate to the Student's Profile → Eligibility tab → Student Subtab → in the Home Language section → click +Add Home Language or click the pencil icon to enter the start, end date, and Language.

Next, add the family's Written and Verbal Communication Preferences. Navigate to the Student's Profile → Eligibility tab → Family Subtab → under the Basic section → click Edit and enter the family's Written and Verbal Communication Preferences.
PLIS Report:
When it comes time for PLIS reporting, CareConnect will pull all data captured for the students and families from the Eligibility tab into the PLIS report.
To generate the PLIS report:
1. Start from the Students Module → Locate PLIS in the Reports section
2. View any already generated PLIS reports or select +Generate to create a new report
3. When clicking +Generate, a pop-up window will open to specify the Program(s), Site(s), and months to report
4. CareConnect may take a few minutes to generate the PLIS report depending on the volume of the data. Once done, CareConnect will list the generated report summary in the list with the information for the report month, sites, and user name who generated or updated the report.
a. Click theaction icon to view the report details
b. Click theaction icon to delete the report if it was generated accidentally
c. Click the action icon to view the log details
5. Within the PLIS details, you will different actions available at all times that are:
a. Student Name hyperlink: This will allow you to quickly open the family record for the student included in the report details to view and/or modify their eligibility information as needed
b. View & Edit: This button is to quickly edit the report details from each cell. This edit will open the different fields that populate the report. NOTICE: Editing here will not update the data back into the Family's Eligibility tab but it may be used if the report is past due and changes needed to be made right away.
c. Export: This button will export the report to a text format that is pre-formatted to follow CDMIS' requirement for an electronic file transfer.
d. View Log: This button is to view any changes made to the PLIS report.
e. Back: This button is to return to the PLIS reports
e. Delete: This button is to delete this generated PLIS report.
