Using CareControl, you will be able to send the CCD-26 to the family all digitally, making the process easier and faster than ever before.

How to Generate the CCD-26

When a CCD-26 is needed to be sent to the family for certification, we will be able to send it directly to the family all digitally through CareControl.

1. First access the family’s record.
   

   

2. Select the case you want to create the CCD-26 for under the Case tab.

3. At the bottom of the selected case, click the Print button.
   

   

4. A new window will appear to format the CCD-26 selections that are relevant to the Case. Fill out the sections as follows:
   
   

1.  Select the Issue Date for the CCD-26

2. Select what Income Source the Family is using.

3. Select on what basis the Family is Eligible for service (Income, CPS)

4. Select what Needs Basis for Parent A

5. Select what Needs Basis for Parent B (optional to account for Single Parent Families)

6. Select Care for children applying

7. Select if Initial or Recertification CCD-26

8. Save
   
   

You will then be taken to a new window to select what form is going to be generated. Click Save and you will be redirected to the Documents Tab to view and send the documents to the family.
