As a subsidy provider, your agency can send documents to your account, and you can send documents back. You'll receive an email letting you know when documents are ready in your CareConnect account. Follow these steps to view and complete them!

Mobile App

Tap Documents.

Documents that require attention will say To Complete

Tap the Document to fill out the eForm. Tap the menu in the corner to Download the document, or to Upload additional documents.

You can send any document or image, even if they have not requested one. Tap Received then tap the + icon:

Your agency may have standard Forms available for you as well.

Website

Documents that require attention will say To Complete

You may be able to fill out an eForm, Download, or Upload documents.

You can also send files using the Add button.
