**Access CareConnect mobile app (iOS and Android) or MyCareConnect Website**
If you did not finish you application, it will be saved as a Draft. Log in to MyCareConnect to resume your application and submit it.
Resume Application:
Click on the Draft button or your application, or click on View Application to open your application:
Continue to add your information. Click Next to find each page, when finished click Submit or save your application to Draft again.
